WELCOME BACK PARENTS AND STUDENTS!

The Nurse’s office will be open on Tuesdays and Thursdays 9:00am-1:30pm and Wednesdays 9am-2pm. The above phone number does not have an answering system, you may access my voicemail by dialing 299-0400 and choosing option #4. Voicemail is available 24/7.  Please call the Main office if your child is going to be absent or late.  We must know if your child will not be in school or will be arriving late.  We will call you at home or at work if your child is absent and we have not heard from you.  This is for your child’s safety.  Please make sure your child is in school everyday, unless they are sick with a temperature of 100 or above, have vomiting, diarrhea or a serious injury. Absences of three or more consecutive days must have a doctor’s note to return to school.  A note from the parent should always accompany a child when they return to school after an absence.  The law allows for 18 unexcused absences. All absences are considered unexcused unless they accompany a doctor’s note. There are several important papers that I must have in order for your child to attend school.  The first is the Emergency Medical Card this should have all numbers that are available for me to reach you in an emergency. Please have a back-up plan ready if your child must be sent home and you are a working parent.  Children may not stay in the nurse’s office for an extended period of time if they are sick.  The second paper is the Health Records Update this form must be filled out each year listing any pertinent medical condition or any medication your child is on, even if it is not taken in school. This form is confidential and kept in a locked cabinet.  You may send this form in a sealed envelope, addressed to me to insure your child’s privacy.  The third form is an updated Immunization Record.  Your child will not be allowed to start school without this form. This form is for new students and those starting Kindergarten and Pre-school.  *PLEASE NOTE* that there are new immunization requirements for pre-school and those students entering grade 6. The fourth form is a current Physical from your child’s doctor. This form is for all new students and those starting Kindergarten and Pre-school. Physicals are good for 365 days prior to your child’s first day of school. Physical forms are due within 1 month from the start of school. State law requires me to prohibit your child from attending school until I receive these forms. The fourth form that is needed is a Physician Medication Order Form.  This form is required only if your child receives medication while in school, and includes over-the-counter medication as well as prescriptions. *PLEASE NOTE* COUGH DROPS ARE NO LONGER ALOWED TO BE BROUGHT TO SCHOOL TO BE DISPENSED BY THE TEACHER OR NURSE. THIS IS A NEW DISTRICT POLICY DUE TO POTENTIAL CHOKING HAZARD.  The medication form is good for the entire school year and all medications must be brought in to school by a parent or responsible adult. The final form is the Self-Administer Medication Form this form must be filled out in order for your child to be supervised taking their medication when the nurse is not present. All forms are available in the nurse’s office.

The Diocese of Camden requires that all adults having regular contact with children must attend a CAP session. This is good for (5) years. Please contact the office for a list of CAP sessions. They will also be posted on the website. You also must have fingerprints on file at the school.  Please see Mrs. Shortencarrier in the main office for the Universal fingerprint form.  This must be done as soon as possible if you are a volunteer at the school. Notice About Food Allergies

 

As per the guidelines of our new lunch program, I must have a list of ANY food allergies that your child has. This must be a note from your child’s doctor. You do not need to make an appointment with your child’s doctor to get the note, simply call and have them fax a note to me (856-299-6556) I need this note by Monday, September 13th.  I must have a note even if your child packs a lunch, in the event that they may purchase a lunch or snack during the course of the year.  This is a MANDATORY policy.  This is for the health and safety of your child. ILLNESSES

 PLEASE NOTIFY THE SCHOOL AND REPORT YOUR CHILD ABSENT

These are a few guidelines to follow:

 

 Children that have a fever of 100 degrees Fahrenheit or higher within the last 24 hour period should stay home from school to avoid the possible spread of disease.

 

 Vomiting and/or diarrhea within the last 24 hours should stay home for a 24 hour period without vomiting or diarrhea.

 

 If your child has a rash that you do not know where it came from, they should be kept home until the cause of the rash is determined.  Many diseases start with a rash before any other symptoms appear.

 

 Severe cold symptoms - excessive coughing or sneezing.  Continued and prolonged coughing may be related to asthma and should be checked by your doctor.

 

 Red, watery, itchy eyes not related to allergies may be a sign of conjunctivitis.  Any drainage from the eyes must be treated. Bacterial conjunctivitis is contagious; children must stay home until medication has been administered for a 24 hours period.

 

Any absence of (3) or more consecutive days, due to illness or injury, must have a Doctor’s note to return to school.  All students must also have a written note from the parent/guardian upon returning to school.

HEALTH ALERT:

As of September 1, 2008, it is mandatory for all Pre-school children to receive the Flu vaccine between September 1st and December 31st. each year.  You must show the school nurse proof of an appointment by October 1st, 2009.

The New Jersey Health Dept is recommending that children in grades k-12 receive an annual influenza (flu) vaccination. Vaccination is recommended because immunity can decrease during the year after immunization and flu strains can change from year to year. Influenza vaccination is the primary method for preventing influenza and its severe complications.  Flu season can start as early as October and extend into May.  The ideal time to receive the flu vaccination is the beginning of October thru December, but an individual can receive it as early as September.   The Center of Disease Control and prevention recommends 2 doses of the inactivated influenza vaccine for children under 9 years of age who are receiving vaccine for the first time.  For adults and older children, one dose is recommended annually. Currently, there are 2 types of influenza vaccines in the market-inactivated and live attenuated influenza vaccine.  Caregivers should consult their child's doctor about the influenza vaccine and which formulation is best for their child.
